Here's where you will make a difference As our Clinical Coordinator, you'll play a vital role in coordinating care and services for our clients with Home Care Packages.
You'll also support our Community Care Management team in various specialty areas such as gerontology, palliative care, hospital-to-home transitions, dementia support, and mental health.
Your responsibilities will include advocating for clients, assessing their needs, and ensuring their well-being.
To be ideal for this role, you will bring the following: Professional qualifications and nurse registration in the ACTKnowledge of health, well-being, and support for older individualsExcellent negotiation, representation, and communication skillsStrong time management, organizational capabilities and computer savvy with MS OfficeOutstanding relationship management skills with clients, medical professionals, service providers, families, and the communityA commitment to good employer practices, including workplace health and safety, equal employment opportunities, and industrial democracy.Australian work rights.
