This role is available on a permanent full-time basis.
Our Clinical Coordinators are responsible for providing clinical leadership, guidance, supervision, and education to all staff to promote excellence in care.ABOUT THE ROLEAs the Clinical Coordinator, you will be responsible for coordinating the delivery of safe, effective, and individualised care to residents and their families (including managing staff and physical resources) and assisting residents to lead an active and meaningful life as independently as possible in keeping with the organisation's Mission, Vision, and Values.You will work with and assist the Care Manager to plan, coordinate, and evaluate the clinical care in the centre to maintain full accreditation and achieve best practice.
The main responsibilities of the role are, but not limited to:To provide clinical leadership in the coordination of nursing and care servicesTo promote and support a person-centered culture and values - respect and treat all people as unique individuals with needs, preferences, and a past historyTo optimise ACFI funding in relation to resident care plans, and ensure that sufficient evidence is maintained for validation of all claimsTo maintain accountability and responsibility for the clinical management of health and care outcomes of the residents in the CentreTo ensure that skills and knowledge appropriate to delegate the task/authority have been considered when delegating health and care responsibilities to other professionalsTo exercise critical thinking within an evidence-based framework to ensure the delivery of high-quality health and care services which promote optimal health outcomes for the residentsTo deputise in the Care Manager's absenceABOUT YOUTo be successful in this role you will have:An understanding of, and empathy with, the Christian faith, values, and ethos of The Salvation ArmyRegistered Nurse - Current unrestricted registration with Australian Health Practitioners Regulation AgencyMinimum four years of clinical experience as a Registered Nurse (preferably in Aged Care)Demonstrated knowledge in the care needs of the older personUnderstanding of and commitment to continuous quality improvementAn understanding of and demonstrated experience with Aged Care Quality Standards and relevant Aged Care legislationAn understanding of and demonstrated experience with Aged Care Funding Instrument (ACFI)Demonstrated excellent organisational and time management skillsHigh level verbal and written communication skillsYou must be a self-starter and able to work autonomouslyStrong IT skills and experience in aged care clinical management using iCareBENEFITSAbove Award wages, access to generous NFP salary packaging of up to $15,900 (save on tax), access to relocation benefits through salary packaging options, access to meal and entertainment benefits of up to $2,650 through salary packaging, paid parental leave, flexible work arrangements supporting you to achieve your work-life balance, access to Fitness Passport, novated car leasing, learning and development to support you in your role, scholarships via Eva Burrows College, supported study options, employee assistance program, career progression opportunities into management, quality etc., and transfer opportunities - we are in every major city in Australia.To apply for this opportunity, please click on the 'Apply' button below to be taken to our online application process.Please note: evidence of the COVID-19 vaccination is a requirement of employment at The Salvation Army Aged Care.Job offers will be subject to a National Criminal History Check as per Police Certificate Guidelines for Aged Care Providers.The Salvation Army is an Equal Opportunity Employer who provides an inclusive work environment and embraces the diverse talent of its people.
