About UsThe Institute for Urban Indigenous Health Ltd was established to provide a coordinated/integrated approach to the planning, development and delivery of comprehensive primary health care services for Aboriginal and Torres Strait Islander populations within the Southeast Qld Region.
The Quality and Innovation team provides clinical oversight and support to IUIH member services and is responsible for the service development and delivery of urban Aboriginal and Torres Strait Islander health and family wellbeing care.
This diverse team includes IUIH Goodna, Policy, and Regional Clinical Support.
About the RoleThe Health and Wellbeing Worker role is designed to guide clients in their autonomous access and use of culturally responsive holistic primary care.
MATSICHS Caboolture Public Health Clinic (PHC) will utilise the IUIH model of care to achieve continuity of care and improve the health status of Aboriginal and Torres Strait Islander people.
The successful applicant will be willing to contribute innovative thinking and be flexible and adaptive to change.
Within this position, you will be required to:
Support clients and families with information, resources, and advice in response to specific requests and/or identified needs, connecting them to services within the IUIH.
Provide education for clients and families on the roles of various providers involved in their care (both internal and external services).
Be the point of first contact in responding to clients and families at times of heightened need to prevent and/or avert episodes of crisis.
Work alongside clients and families in their goal setting and prioritisation of health and social needs.
Implement plans based on goal setting in collaboration with the core multidisciplinary team.
About You
Current C Class Drivers Licence (QLD) Valid QLD Blue Card – Working with Children Check Current Criminal History Check Demonstrate an understanding of issues affecting Aboriginal and Torres Strait Islander people and their health.
Ability to communicate sensitively and effectively engage with the community built on mutual respect and regard.
Desirable, but not mandatory
Certificate/Diploma in Aboriginal and Torres Strait Islander Primary Health Care.
