Job DescriptionKalwun Development Corporation Ltd is an Aboriginal & Torres Strait Islander community controlled organisation currently seeking to fill the following vacancy within our Health and Wellbeing Services division.
Kalwun Health and Wellbeing Service delivers a primary health care service to the people of the Gold Coast & surrounding communities.

We are looking for a motivated Health and Wellbeing Worker who is passionate about improving the health and wellbeing of Aboriginal and Torres Strait Islander people.
The successful candidate will provide a high-quality service along with contributing to the development of models of holistic health incorporating continuity of care.
As part of Kalwun's ongoing growth and development, you will be joining a community-focused team at the Coomera clinic.
Position Duties Connecting and yarning with clients and families to the service to collect, collate and update their stories.Providing health information with education to clients and families which promotes autonomy and choice.Working alongside clients and families in their goal setting and prioritisation of health and social needs ultimately implementing effective plans.Responding to clients and families at times of heightened need to prevent and/or avert episodes of crisis.Making a positive contribution to illness prevention and promotion of health through chronic disease management, health screening, health assessments, recalls and patient education.Holding a relevant Certificate III Qualification (Aboriginal and Torres Strait Islander primary health care, Community Services, Allied Health Assistance or equivalent).Demonstrating an understanding of issues affecting Aboriginal and Torres Strait Islander people and their health.Communicating sensitively and effectively with the community built on mutual respect and regard.Demonstrating ability to meet targets and performance outcomes.Exhibiting demonstrated computer literacy including proficient keyboard and data entry skills.Working effectively as part of a multi-disciplinary team.Holding a current Qld C class drivers licence.What we offer
