We are looking for an experienced Support Coordinator/Case Manager with a focus on physical, complex health needs, with less emphasis on mental health.
You will be proactive in meeting the needs of our clients and anticipating the changes in health status and the need for reviews and assessments.
You will have a working knowledge and understanding of the physical health changes that come with chronic health conditions and will be able to preempt what is required for the client and their families.
The position will offer a variety of support coordination and case management.
Part of the role will be to coordinate the onsite overnight assistance for our Hope Island Project.
This position is predominantly office-based in our Head Office located in Southport.
In this role your day-to-day responsibilities will include but are not limited to: Being client-centred is of utmost importance to us, so you will have excellent customer service skills with a large part of the role supporting and communicating directly with participants, their families and other support networks to provide information and understanding of their NDIS plan.Comfortable working as part of a multidisciplinary team with the ability to effectively advocate for your clients.Complete implementation, progress and annual reports for your clients.Plan and coordinate participants to access support services in accordance with their allocated NDIS funds/budgets.Ability to advocate for your clients and their needs.Some travel will be involved in the position as we service clients from the Sunshine Coast to Northern NSW.

To be successful in the role you: Experience as a Support Coordinator and/or Case Manager.Degree level qualification in relevant field: social work, psychology, allied health, nursing or community services.Experience working with and coordinating support for people with complex disabilities with high intensity care requirements.Ability to work collaboratively in our multidisciplinary team.Highly developed verbal and written communication skills.Confidence in managing your time effectively, including an ability to work autonomously and as part of a team.Well-developed interpersonal skills that you use to establish excellent working relationships with both internal and external stakeholders.Comfortable using technology.Blue card and NDIS Workers Screening Clearance or willingness to obtain.Current Australian Driver's Licence.Be available to participate in the On Call roster, once every 8-10 weeks (and paid a generous on-call allowance).If you are looking for a change or want to do something fresh, come join an organisation where your exceptional skills are appreciated
