About the Role Reporting to the Coordinator Social Inclusion and Connection, the Senior Team Leader Social Inclusion supports a team which engages community development approaches to support opportunities for older people to remain active and healthy in community life.
Responsibilities include: • Lead key activities to progress Council's Age Friendly Darebin commitments • Measure Council funded programs and roles to establish impact and outcomes for the community • Carelink plus (CL+) optimisation - Map how all roles in the team can better document and produce automated reports.
And the migration of the Aged Housing Sponsorship Program to CL+ • Progress projects exploring pathways to support new and emerging older adult groups • (5) Strengthening internal partnerships with Council's Social Support Program (SSP) – a Commonwealth Home Support Program (CHSP) to support an enhanced client experience.
• Oversee the day to day and strategic management of the team, programs, facilities and budget Skills & Experience • A tertiary qualification in Community Development, Social Sciences or Allied Health (or related discipline) and/or relevant experience in the social work/community development environment.
• A current Victorian Driver's Licence (essential) • Project management skills • Excellent listening skills and well-developed writing skills • Sound communication skills and an enjoyment for working with community • Experience in using a participatory or co-design framework to inform program design and new initiatives.
