About the Aged Care Assessment Officer (Comprehensive) role We are seeking Clinical Assessors with recent ACAS/RAS assessment experience and tertiary qualification in Allied Health, Nursing or Social Work to:
Conduct comprehensive care needs assessments for individuals aged 65+Develop and review care plans to support independence and wellbeingTo be considered for this role, you will have: Relevant tertiary qualifications and current professional registration (if applicable).At least one year of experience conducting ACAS/RAS clinical assessments.Proficiency in using the My Aged Care Portal and a solid understanding of the Integrated Assessment Tool (IAT).A demonstrated commitment to ongoing professional development and evidence-based practice.Exceptional relationship-building skills to effectively engage with clients and empower them in their healthcare journey.Current Driver's License - travel required.YourCH has fostered an organisation-wide supportive and diverse team that allows staff to bring their whole self to work, both personally and professionally.
