The Patient Flow Management team is seeking to engage a Clinical Nurse Consultant in the role of Continuing Care Coordinator.
The role provides a specialized nursing consultancy service to patients, carers, and other health care professionals in the separation planning and continuum of care process at Nepean Hospital.
This includes a timely facilitation, coordination, and planning of a person-centred, safe transfer of care for patients referred through e-Orders.
In this position, you will serve as a clinical expert who through leadership facilitates evidence-based practice.

What you will bring to the role:
Current registration with the Australian Health Practitioner Regulation Agency (Ahpra) as a Registered Nurse with a minimum of five years' experience in an acute setting and two years clinical experience in specialty.
Demonstrated effective clinical communication using information technology and sound written, verbal, and interpersonal skills.
Demonstrated ability to work effectively and collaboratively as part of the Multidisciplinary Health Care team.
Proven ability to prioritise and organise own workload.
Demonstrated ability to network and liaise with government and non-government service providers.
Demonstrated ability to apply clinical expertise in patient-centred care consultancy with the ability to work independently.
Consistently demonstrates behaviours that reflect the CORE Values: Collaboration, Openness, Respect, and Empowerment of Nepean Blue Mountains Local Health District.
Understanding and knowledge of current NSW Ministry of Health Policy Directive Admission to Discharge Care Coordination and how this guides practice.
