We understand the physical and emotional challenges when an aging loved one requires extra help to remain in his or her own home.
Position Overview:
As a Disability Support Worker, you will be essential in providing high-quality, person-centered care and support to our clients.
Your responsibilities will include helping clients navigate the NDIS system, offering personal assistance, encouraging community involvement, and addressing accommodation needs.
Key Responsibilities:
Personal Care & Support: Assist with daily activities such as personal hygiene, dressing, feeding, and mobility, promoting dignity and independence at all times.Social & Community Participation: Support participants in accessing community-based activities and opportunities that align with their interests and goals, promoting social inclusion and engagement.Developing & Implementing Care Plans: Work collaboratively with participants, their families, and other support professionals to develop individual care plans and assist with the achievement of personal goals.Health Monitoring & Wellbeing: Administer medication, monitor health conditions, and ensure that participants' overall well-being is managed effectively.
Liaise with healthcare professionals when required.Documentation & Reporting: Maintain accurate and detailed records of care provided, progress, and any incidents, in line with NDIS guidelines and legal requirements.Team Collaboration: Work alongside other support workers, therapists, and healthcare professionals to ensure coordinated, person-centered care
Essential Qualifications and Skills:
Certificate III or IV in Disability Support (or equivalent)Previous experience in disability support, aged care, or a related field (highly desirable)Strong interpersonal and communication skills, with the ability to build rapport with participants and their familiesA compassionate, patient, and empathetic attitude towards people with disabilitiesAbility to work independently and collaboratively within a teamFirst Aid and CPR certification (preferred or willing to obtain)A valid National Police Check and Working with Children Check (or willingness to obtain)A current driver's license and reliable transport (depending on the role and location)
