Looking to join a team coordinating care and services to support patients in their own home?
We have a part-time opportunity available to work within the SA Community Care (SACC) and the My Home Hospital (MHH) program.
Using your clinical skills, you will ensure a comprehensive client-centred management plan is achieved for our clients together with:
Conducting clients initial phone assessment, care planning and goal settingCoordination of in-home services to support clients receiving care at homeBuilding and maintaining effective relationships with a variety of different stakeholdersMonitoring patient progress and updating care plans as appropriateLiaison with healthcare professionals and specialistsDischarge planning including referrals to long term servicesProvision of telephone support, education, and adviceOur Case Coordinators work a rotating 7-day roster between the hours of 7am & 8pm.
You will be rostered to work alternate weekends in the comfort of your own home.

What will set you apart from the rest?An experienced clinician, you are customer focused and driving to deliver best practice outcomes across our programs.
Degree qualified as a RN, it is essential you bring a clinical background in an acute hospital setting.
3-5 years' experience acute hospital experience as a Registered Nurse with unrestricted AHPRA registrationRecent experience in care coordination (highly desirable)Strong customer service orientation and relationship management skillsExcellent clinical reasoning skillsAbility to work in a fast-paced environment.Proficient computer skills to enable fast and accurate data entry into patient electronic medical records.Our Vision is to be the healthiest workplace in Australia by 2030.
