You will be working with a leading Community Care Service provider, based in Kilsyth.
Joining a team of dedicated, passionate and very progressive colleagues and management, you will be reviewing and updating the care plans of a caseload of elderly clients who wish to remain living independently in the community, assisting them to achieve their independence goals and use their CHSP or HCP funding to its full potential by accessing care services, allied health, social support and nursing services.
Starting ASAP and working full time, you are a confident and experienced case manager or Assessor who is able to take on new processes and software quickly and is open to new ideas and ways of working.

What you need:To work with McArthur you must have:
Relevant experience in CHSP Assessment or case management of clients on home care packagesQualifications in Nursing, Allied Health or social scienceAble to commit to 4-5 days per week (Mon - Fri) starting immediately.Strong knowledge of CHSP and HCP legislation and fundingExperience working with a care management software packageVictorian Drivers Licence and reliable car and willing to travel to complete home visits across the eastern suburbs #J-18808-Ljbffr
