Care Manager ad - LGC Oxley We have an exciting opportunity for a Home Care Package (HCP) Care Managerto join our team in Oxley, Qld, offering telephone-based care for our clients. myHomecare Group is a leading Home Care Package Provider in Australia.
Joining our company, you will be given opportunities to grow and progress your career in the aged care industry.With the introduction of Support at Home in July 2025, it is an exciting time to develop your care management skills and increase your knowledge of aged care.
This position will be in our fully self-managed, part-managed or fully managed care teams.
We cater our support to each client's unique care needs to ensure we have the best solution for every Home Care Package holder.
What will we offer you? 3 days working from the office and 2 days working from homeTelehealth model of care - no travelling to client's homes or being caught in trafficFull time, permanent employment, 9-5pm weekdaysInduction program to properly educate you in our processes and systems plus ongoing training on the jobTeam events to celebrate our success and foster internal relationshipsA fun and supportive team atmosphereAs a Care Manager, you will: Deliver excellent telephone-based client care for elderly Australians including Care Planning, Assessment and CoordinationUse your customer service and aged care management skills to make a mark on the communityDevelop, monitor, and reassess care plans in consultation with clients, their families and other healthcare professionalsWork in collaboration with our Nurses and other Managers to facilitate, develop and shape our service offeringsMonitor the budget of care packages and ensure clients understand service feesEngage with internal teams to facilitate client on-boardingEnsure compliance with all relevant legislative and industry standardsWe are looking for someone with: Excellent verbal and written communication skillsStrong computer skills in CRM systems, database management, Microsoft OfficeThe ability to work at pace with multiple clients and stakeholdersExperience in aged care with knowledge of Home Care Packages and the Aged Care Quality Standards is desiredNursing qualifications are desirable, but not essentialThe right to work in Australia and obtain a National Police ClearanceApply now to discover how you can develop your skills while truly helping people in your community.
