Role Overview
As a Client Liaison Officer, you will at times be the primary point of contact for clients, their families, and healthcare professionals.
You will ensure that clients receive the appropriate care services based on their individual needs and preferences by using your administrative skill.
This role requires a proactive, empathetic, and organised individual who can build strong relationships and provide ongoing support to our clients.
Working from your home in metropolitan Perth, the role requires a person who can be adaptable and flexible, with an ability to prioritise work.
You will frequently be required to conduct face to face visits with clients, so your ability to communicate well is essential.
Key Responsibilities:

Act as the first point of contact for clients, families, and healthcare providers.
Conduct client assessments and develop personalised care plans in collaboration with healthcare professionals.
Coordinate and schedule care services, ensuring clients' needs are met in a timely manner.
Maintain regular communication with clients and their families to monitor satisfaction and address any concerns.
This will be via phone conversations and face to face meets.
Assist with the recruitment and onboarding of care staff, ensuring they match client requirements.
Liaise with external organisations (ie referrers to our services and healthcare providers) to ensure seamless care coordination.
Maintain accurate records of client interactions, care plans, and service updates.
Provide support in resolving client issues and complaints, ensuring a high level of service.
Stay updated on industry regulations and best practices to ensure compliance with care standards.
Essential Requirements:

Experience in a client-facing role (experience in community care is a plus).
Excellent communication and interpersonal skills.
Strong organisational skills with the ability to manage multiple tasks simultaneously.
Compassionate, empathetic, and able to work with vulnerable individuals.
Ability to work both independently and as part of a team.
Proficiency in MS Office / google suite of products and care management software (required).
Knowledge of community care services is advantageous.
Full Australian driving license with no restrictions (required).
Full working rights in Australia.
