The Drug and Alcohol Youth Service (DAYS) in East Perth delivers critical support to young people and their families through a holistic range of free and confidential alcohol and other drug services.
As the only youth-specific withdrawal unit in Western Australia, DAYS provides a safe, supportive, and structured environment for young people undergoing rehabilitation.
The Aftercare Case Manager plays a vital role in ensuring continued progress for individuals who have completed the residential program, helping them sustain positive outcomes, minimise relapse, and address their physical, social, and emotional needs as they transition back into the community.
Your Key Responsibilities Provide quality case management practices tailored to the individual needs of young people, ensuring services are culturally safe and aligned with Mission Australia's National Case Management Approach.Build strong, trusting relationships with young people to facilitate their engagement in treatment and support.Collaborate with clients to create holistic, strength-based, solution-focused case/support plans using SMART goals, and facilitate referrals to suitable programs and agencies as needed.Assist young people in transitioning from the service to independent living or other support services, building their capacity to self-manage and access resources.Advocate for young people, provide referrals to appropriate services, and deliver harm minimisation education sessions to clients, families, external agencies and the community.Prepare and update case notes, assessments, risk management plans, statistics and reports while ensuring compliance with program requirements and prompt data entry into relevant databases.Build and maintain strong positive external agency networks, participate in community sector meetings, and contribute to collaborative care and management for at-risk youth.Provide mentoring and coaching support to team members, actively engage in continuous quality improvement processes, and participate in supervision with the Program Manager.Requirements for Success A Degree or Diploma in Social Welfare, Social Science, Youth Work, or a related field, or have significant experience in Alcohol and Other Drug (AOD) services, youth work, or case management.Demonstrate a strong understanding of child and adolescent development, youth safety practices, and the complexities of AOD-related issues facing young people.Show experience managing a caseload of clients with complex needs, including applying a case management approach to develop and implement individualised support plans.Possess excellent interpersonal and communication skills, with the ability to engage, motivate, and maintain professional relationships with young people and external stakeholders.Exhibit strong time management, organisational skills, and a high level of competency in maintaining client records, writing reports, and using computer-based programs, including the Microsoft Office suite.Demonstrate flexibility, perseverance, and innovation in outreach engagement with at-risk youth, including addressing vicarious trauma and maintaining motivation in clients accessing voluntary services.Capable of working independently and as part of a team, contributing to a supportive and productive work environment while managing the demands of outreach and autonomous responsibilities.Full driver licence with no restrictions.Before starting work with us, you will need to undertake a national police history check, qualifications, and referee checks, and have a clearance to work with children/vulnerable people.
