The Role The vision for South Eastern Sydney Local Health District (SESLHD) is ‘exceptional people, healthier lives’. Provides support and care coordination to all Aboriginal and Torres Strait Islander people referred to Prince of Wales Community Health, working in partnership with the clinical care providers in Community Health; implements a range of health promotion activities; Participates in the review and development of existing and new programs; Provide culturally appropriate care and support for Aboriginal people and their families and support the decisions of the client, by encouraging them to navigate their own health care journey, identify barriers and find solutions to overcome these, that will assist in better health outcome delivery Where You'll be Working Prince of Wales Hospital and Community Health Service is a Level 6 Tertiary Referral Hospital with an inpatient bed base of 370. Each year we care for more than 70,000 patients in our Emergency Department and have around 50,000 admissions to the inpatient units. POWH offers an outpatient service and rural outreach service and provides more than 900,000 occasions of non‑admitted patient care each year, including innovative virtual models of care. We have transitioned the majority of inpatient units and wards to the Acute Services Building (ASB) including a state of the art Emergency Department and Intensive Care Unit. Selection Criteria To be considered for this position, please ensure you address the below questions as thoroughly as possible. Being of Aboriginal &/or Torres Strait Islander descent is a genuine occupational qualification for this position under Section 14 of the Anti-Discrimination Act 1977 (NSW). Holds or aspires to hold a minimum Certificate III in Aboriginal Primary Health Care Knowledge and understanding of significant issues that impact on the health and wellbeing of Aboriginal people and their families Demonstrated skills in health education and care coordination and experience working as an Aboriginal health worker Demonstrated computer literacy Demonstrated oral and written communication skills and demonstrated ability to work as part of a team Current NSW Drivers Licence Class C Ability to provide proof of Aboriginal or Torres Strait Islander descent?
