This full-time or part-time fixed-term role (until June 2027) offers the chance to be part of a major system-wide transformation, with some evening and weekend shifts included. We are seeking enthusiastic, motivated and forward-thinking Grade 4 Allied Health and Pharmacy professionals (Dietitian, Exercise Physiologist, Occupational Therapist, Pharmacist, Physiotherapist, Podiatrist, Social Worker or Speech Pathologist) to join the Acute Frailty Team (AFT) as we expand across the Monash Health. You will work closely with a team of Advanced Allied Health Geriatric Practitioners, Emergency Department Geriatricians, Care Coordinators and the broader ED team to deliver impactful, patient-centred initiatives. This is an exciting chance to contribute to a high-priority strategic project, influence models of care across the organisation, and help improve outcomes for some of our most vulnerable community members. You will play a central role in providing expertise in patient centred care for frail older people presenting to our ED's, with a strong focus on reducing avoidable hospital admissions and minimising the acute length of stay. Key tasks will include: Provide early intervention in the Emergency Department and Short Stay Unit aiming to avoid hospital admissions using a home first approach Utilise expertise in Geriatrics and Frailty, and extensive specialist knowledge of home based, community and residential services to navigate complex service systems to provide exceptional care to older people, ensuring the right care is provided at the right place and right time.
