About the Role We are seeking a capable and community‑minded Elder Care Community Coordinator to lead our local Elder Care Support team in Midland. In this role, you will: Supervise and support Connectors and provide daily direction to Drivers Oversee Elders’ end‑to‑end aged care pathways Troubleshoot barriers and ensure culturally safe navigation Build strong relationships with assessors, aged‑care providers and local services Ensure accurate data entry, file quality and compliance Prepare monthly Site Data Packs (KPI tables, risks, issues, vignettes) Manage rosters, transport schedules and workloads Contribute to continuous improvement and safe outreach practices Champion privacy, WHS, cultural safety and safeguarding policies This role is perfect for someone with strong community connection, excellent organisational skills, and a passion for improving the aged‑care experience for Elders and families. What You’ll Bring Essential Experience supervising staff in community or health settings Ability to work with Elders, families and community in culturally safe and trauma‑informed ways Knowledge of My Aged Care pathways (or ability to learn quickly) Excellent communication, advocacy and system navigation skills High standard of documentation, data quality and record‑keeping Proficiency with Microsoft 365 and case management systems WA Driver’s Licence, National Police Clearance, WWCC, vaccinations Desirable Experience in an ACCHO or aged‑care navigation role Knowledge of Elder ECHO, ACRAS/ACAT pathways Experience delivering micro‑trainings to staff Mandatory Qualifications Qualification in Community Services (minimum Cert IV) Aboriginal Mental Health First Aid (or willingness to obtain) Why Join MKAC?
