The opportunity IFYS has an exciting opportunity for two Transition Case Managers to join the Fresh Start team. The Transition Case Manager supports young people (15–24) involved with Youth Justice or Community Corrections to achieve their self‑identified education, training, and employment goals. The role involves assessing referrals, developing individual plans, connecting with employers, managing risks, and collaborating with stakeholders to improve well‑being and community engagement. What's on offer? Two fixed part‑time positions, contracts ending 30th October 2026 Minimum 60 hours per fortnight, working hours between 8:30am and 4:30pm Salary base $88,099 per annum pro rata + superannuation – IFYS is committed to gender pay equity $44.58 part‑time hourly equivalent Based in Maroochydore Excellent not‑for‑profit salary packaging options (package up to $15,900 per FBT year) Motor vehicle allowance $185 per week Join a supportive, inclusive and vibrant team Paid in‑house training opportunities including : Therapeutic Crisis Intervention and the Sanctuary Model Free access to the CALM Premium App & Flight Centre personal travel discounts Access to the Employee Assistance Program (EAP) The key responsibilities required for this role include: Receive and assess referrals, determining eligibility and priority of response Support young people to identify their needs and goals, through the development and implementation of individualized goal plans including the management of brokerage funds Support young people to obtain job and training readiness skills and access pathways to achieve education, training and employment goals Connect with local business and employers to identify work experience and employment opportunities Identify and assess risk level for young people and respond appropriately, engaging other support services when required The timely and accurate completion of administrative duties, case notes, case plans, data collection, financial management, time sheets and incident reports in line with legislative and contractual requirements Work collaboratively with key stakeholders including Youth Justice and Community Corrections Advocate on behalf of individuals to ensure that services accessed and provided are relevant, flexible and accessible. Contribute positively to the development of resources, work procedures and processes, funding submissions and other projects Requirements for the role: Professional experience working with young people in a therapeutic capacity Diploma or tertiary qualification in a relevant area of study will be considered favorably Excellent written and verbal communication skills, including report writing Demonstrated knowledge of strengths‑based and client‑centred practice Demonstrated strong cultural knowledge and understanding of First Nations communities, with the ability to engage and support young people in a culturally sensitive and respectful manner Must hold or be willing to obtain if successful: Current Queensland driver’s license with no restrictions, own reliable transport QLD Working with Children Check (Paid Blue Card) As part of our recruitment process, we do a number of history screenings including a Licensed Care Suitability check (LCS2) and a National Crime Check Ability to provide two references from recent employers (including your current line manager) We are an inclusive workplace that celebrates diversity and difference in our communities.
