Aboriginal Health Practitioner / Aboriginal Health Worker - Identified 2 days ago Be among the first 25 applicants Employment Type: Temporary Part Time until November 2026 Position Classification: Aboriginal Health Practitioner / Aboriginal Health Worker Remuneration: $31.52 – $45.57 per hour + super Hours Per Week: 28 Location: Wonnarua Country | Cessnock Community Health Service Requisition ID: REQ Applications Close: Sunday, 23rd of November About the role: You will work alongside the Clinical Nurse Consultant of the Integrated Chronic Care for Aboriginal People Program (ICCAPP) and collaborate with internal and external partners to provide culturally sensitive, holistic services. The role involves clinical assessment, screening, case management, education, and referral for Aboriginal and Torres Strait Islander clients. You will support families and communities and maintain health and wellness for Aboriginal people in the Lower/Upper Hunter sectors. About Us: ICCAPP is a community outreach service that increases access to culturally appropriate health services for Aboriginal and Torres Strait Islander people aged 15 years and older who are at risk of or have chronic disease. The program aims to improve health outcomes by working in partnership and developing flexible local approaches. About You: Motivated and organised professional with deep understanding of Aboriginal and Torres Strait Islander cultures. Confident building respectful relationships, communicating effectively with individuals, families, and communities, and delivering culturally safe care. Requirements: Holds a Certificate IV in Aboriginal Primary Health Care (Practice) and a current authority to practice as an Aboriginal Health Practitioner with AHPRA, or holds a minimum Certificate III in Aboriginal Primary Health Care (or a Certificate III health qualification in the relevant area) and is willing to undertake a Certificate IV within 18 months to transition to Practitioner status with current AHPRA registration. Eligible to drive in NSW and able to travel as required.
