About Us The Integrated Chronic Care for Aboriginal People Program (ICCAPP) is a community outreach service that aims to increase access to culturally appropriate health services for Aboriginal and Torres Strait Islander people aged 15 years and older who are at risk of or have chronic disease. The program is based at Cessnock District Hospital Campus, providing services to Aboriginal people who reside in the LGA and communities served by the Lower/Upper Hunter Sectors. The ICCAP team consists of a Clinical Nurse Consultant and an Aboriginal Health Practitioner and is managed by the Nurse Manager of Cessnock Community Health Service. About the Role As an Aboriginal Health Practitioner / Aboriginal Health Worker you will work in conjunction with the Clinical Nurse Consultant ICCAPP and collaborate with both internal and external partners to deliver flexible, holistic, culturally sensitive services for Aboriginal clients. Key activities include liaison with community groups, health professionals and partner organisations, clinical assessment, screening, case management, education and referral for Aboriginal and Torres Strait Islander people accessing the ICCAPP service. Responsibilities Conduct clinical assessment and screening of Aboriginal clients. Manage case plans, including education and referral to appropriate services. Provide culturally safe and responsive care in collaboration with the ICCAP team. Liaise with community groups, health professionals and partner organisations. Support Aboriginal clients who have been discharged from hospital and require chronic disease management. Assist in data reporting and use of health record systems. Requirements Hold a Certificate IV in Aboriginal Primary Health Care (Practice) and current authority to practice as an Aboriginal Health Practitioner with the Australian Health Practitioner Regulation Agency (AHPRA). Or hold a minimum Certificate III qualification in Aboriginal Primary Health Care or a Certificate III health qualification in the relevant area of care, and be willing to undertake a Certificate IV within 18 months to enable transition to Aboriginal Health Practitioner status. Eligible to drive in NSW and willing to travel as required. Strong understanding of Aboriginal and Torres Strait Islander cultures. Demonstrated ability to build respectful relationships and communicate effectively with Aboriginal communities.
