PositionProfile:We are seeking suitably experienced,motivated & innovative team members, with excellentinterpersonal & organisationalskills to join the WACountry Health Service (WACHS) Pharmacy Services team. Regional Clinical Pharmacists assist in thepromotion of safe, rational and cost effective drug therapy throughthe provision of clinical pharmacy services for hospital patientsthroughout the region. The services provided are to include but arenot limited to completing the medication reconciliation process foradmitted hospital patients, dispensing medications to patients,providing drug information to regional nursing and medical staffand making independent decisions on therapeutic strategies tooptimize patient care. The Senior Pharmacists are responsible fordelivering safe, effective and high quality care through theprovision of virtual clinical pharmacy services for patientsthroughout WACHS. These services include but are not limited to;virtual pharmacy for inpatients at WACHS sites without on-sitepharmacist support, virtual training and upskilling of juniorpharmacists in the regions, pharmacist led virtual outpatientclinics in the areas of visiting specialists, pre-admission, dayinfusions and palliative care. These positions will work closelywith the WACHS-wide Pharmacy Services team and regional pharmacydepartments to ensure best practice and patient centric care isdelivered utilising modern, innovative and technologically advancedservices. Eligibility to Apply: To beeligible for appointment, applicants must have a valid working visa(for fixed term appointments) or be an Australian citizen, permanent resident, or have a visa to live and work indefinitely inAustralia (for permanent appointments).
