Job Summary An exciting opportunity has arisen for an enthusiastic & motivated individual to join our award‑winning Reactive Emergency Assessment Community Team (REACT) as a Healthcare Support Worker. REACT is based in the community and assesses and supports patients experiencing a short‑term crisis, helping them to remain in their own homes and avoid an emergency hospital admission where possible. The integrated, multi‑agency team covers a varied caseload and includes nurses, physiotherapists, occupational therapists, doctors, healthcare assistants, social workers and staff from Suffolk Family Carers, Citizens Advice and the British Red Cross. Key Responsibilities Develop and maintain a flexible, proactive and positive approach to the role, being innovative and open to change in working practices. Take responsibility and ownership of high‑quality service delivery, fostering a culture of integrity and acting as an ambassador for REACT. Manage an effective interface with colleagues from health, social care and voluntary care services to ensure an integrated approach to patient‑centred care. Support clinicians by completing delegated tasks, meeting the individual needs of patients and service users. Work in collaboration with patients, carers, healthcare professionals and agencies to ensure a joined‑up, integrated approach to patient care. Support end‑of‑life and long‑term life‑limiting conditions, aiming to reduce unnecessary admissions to hospital or long‑term care. Provide one‑to‑one support in patients’ homes, supporting all aspects of personal care and daily activities using a reablement approach. Working Hours & Shift Patterns The service provides 24‑hour support with early, late, night, weekend and bank holiday shifts. The role requires a valid UK driving licence and the ability to travel to patients’ homes using either your own car or the team’s pool cars across Ipswich and East Suffolk, with a central base in Ipswich and planned extensions to Woodbridge and Stowmarket. Remuneration & Contract Band 2 (minimum 12 months care experience, no qualifications required – Accelerated Care Certificate required for progression) or Band 3 (minimum 6 months NHS experience and NHS Care Certificate). £24,465 to £26,598 a year per annum, pro rata. Permanent, full‑time contract. Qualifications & Essential Skills NHS Care Certificate. Evidence of completed competencies as part of a professional portfolio. Level 3 vocational qualification or equivalent experience. Ability to understand inter‑professional working and boundaries across health, social care and voluntary services. Basic knowledge of Microsoft Word and email. Ability to travel between locations. Clear verbal and written communication skills. Desirable Skills & Qualities Functional skills or willingness to undertake. Willingness to undertake further study. Provision of person‑centred care. Understanding of risk management, health and safety issues, including safe manual handling. Understanding of the nature of the carer’s role in fulfilling basic human needs. Knowledge of relevant legislation, clinical procedures and observations.
