As the Bereavement Support Worker, you will be the clinical lead for the Bereavement Support in the Community service. The Social Worker/ Psychologist is an integral part of the MLHD Community Care Team and will work within the Palliative Care Team and closely with the Community Care Nursing team to identify and link clients who are receiving or have received Palliative Care services and require bereavement support. As the Bereavement Support Social Worker/ Psychologist, you will be responsible for: Coordinated support and bereavement follow up of family members/carers of patients of the MLHD Palliative Care Service. Consistent provision of information and psycho-education for bereaved family members/carers of patients of the MLHD Palliative Care Service. Bereaved family members/carers have awareness of the available support services and resources, including the availability of bereavement counselling through MLHD for family members/carers of patients of the MLHD Palliative Care Service who meet referral criteria. Ensuring client/patient care is delivered within professional, organisational, legal and ethical boundaries and reflects evidence-based practice and knowledge and MLHD values. Our Ideal Candidate Will Demonstrate Tertiary qualification in Social Worker with eligibility for membership of the Australian Association of Social Workers OR AHPRA registration as a Psychologist Advanced Social Work/ Psychology clinical knowledge, skills and experience, with an emphasis on evidence-based practice. Well-developed time management skills, written and verbal communication, and IT skills including an ability or willingness to use telehealth as a mode of delivery of clinical care.
