Physiotherapist – Grade 2 Western Health – Sunshine Hospital (Sunnybank) – Full‑time, ongoing. About the Role We are seeking a passionate and motivated Grade 2 Physiotherapist to join the Western Health at Home Division. In this rewarding community‑based role you will provide physiotherapy assessment and management to patients referred to services such as the Community Based Transition Care Program (TCP), Rapid Allied Health (RAH) and other Western Health at Home programmes. You will work across a variety of services, supporting patients to achieve their goals and maximise independence in their own homes and communities within a collaborative, multidisciplinary team committed to high‑quality, patient‑centred care. The role involves providing physiotherapy assessment and management in patients’ homes, in clinic or via Telehealth to support safe discharge, prevent complications and reduce hospitalisation. The caseload will be varied and may include musculoskeletal conditions, age‑related issues (e.g. falls), neurological and vestibular conditions, and other complex presentations. About You Qualified Physiotherapist with current or eligible AHPRA registration Minimum of 2 years’ clinical experience in relevant acute, subacute and/or community settings Excellent communication and time‑management skills Collaborative team player who also enjoys working autonomously in patients’ homes Strong commitment to delivering high‑quality care to patients in their own homes Current Victorian driver’s licence About Western Health Western Health is one of the largest healthcare providers in Victoria, caring for a vibrant, diverse and rapidly growing community of one million people in Melbourne’s western suburbs.
