About the Role Join our team as a Team Leader – Proactive Management and Support on a full time, fixed term role until February 2025 covering parental leave. We are seeking a passionate, experienced Allied Health Clinician with excellent communication skills to join the Proactive Support and Management leadership team. The Team Leader performs a key clinical leadership role in supporting the program outcomes and management of staff as directed by the Program Manager, to achieve the goals and directions set out by Monash Health and Department of Health. This role is integral to bridging the gap between acute and community services. It requires experience within acute settings as well as an understanding of community services. The role works across various Monash Health sites. We operate from a person‑centred care framework and align with the social model of health. As part of the role there is a requirement to travel to other Monash Health sites, so you’ll need a valid driver’s licence. About Us Monash Health Community proactively supports our community to improve and maintain health, independence and well‑being by coordinating and/or providing integrated, multidisciplinary care across multiple settings within the local community. Our aim is to empower and prepare clients to self‑manage their health and health care. Working with clients across the aged spectrum, Monash Health Community provides a continuum of care, supporting clients through all stages of non‑acute care from prevention through to rehabilitation. We provide services that support clients in their homes, community‑based facilities and dedicated Monash Health Community hubs. About You You are a dynamic clinician with a genuine passion and interest in this unique opportunity, have been developing your leadership skills, and are ready to take this next step in your career. You lead by example, thrive working in a dynamic environment, and are truly passionate about delivering a high standard of evidence‑based care to all clients. You have sound skills in assessing and planning patients’ community health discharge requirements and have knowledge of the various community health support services available.
