Our values: Trust Respect Empathy Open Minded Integrity ABOUT THE ROLE Due to our continued growth, we have an exciting opportunity for a motivated, self-directed, and experienced Clinician to join our team as a Care Manager, reporting directly to a Regional Care Manager. A Registered Nurse by profession, the Care Manager is a key role responsible for the oversight of care and services to clients This rewarding and hands on role requires a proactive approach to the planning, coordination, delivery, and care management of clients and services. Working closely with the Rostering Team, Client Liaison Team and the role includes engaging with and leading our care team, consisting of Nurses and Carers. Full-time position available in Mornington We are looking for people who have: A current registration with AHPRA as either a Division 1 Registered Nurse Registered Nurses with at least 5 years experience in the field. A passion for caring for people and delivering a high standard of care. Excellent communication, customer service and interpersonal skills. Exceptional organisational and problem-solving skills. Access to a smartphone. A current driver’s license and access to a registered, insured and well-presented car.
