We are seeking a motivated individual who wishes to join us. This position is countywide and not just the Cambridge area. To ensure the role is fulfilled it is essential that the successful candidate is able to travel around the county without using public transport and hold a valid UK driving licence and use of a personal vehicle. Main duties of the job Please note for this role you will be required to travel independently around the county meeting strict time deadlines. To provide an advanced standard of clinical assessment, evidence‑based advice and education to prevent and manage wounds for patients, carers and healthcare professionals. To support the tissue viability team with the allocation of beds and pressure‑relieving equipment for ICES within Cambridgeshire. To provide an advanced standard of clinical assessment, evidence‑based advice and education to individuals with tissue viability problems. To work as part of the multidisciplinary team with the aim of preventing admission, supporting early discharge and providing ongoing or shared care for patients with wounds. To undertake all aspects of clinical duties as an autonomous practitioner with access to advice and support from senior colleagues. To participate in evidence‑based audit and research projects. To plan, implement and evaluate individualised specialist patient care programmes based on evidence‑based practice and the application of clinical reasoning skills. To provide supervision to health care professionals in relation to prevention of pressure ulcers/Moisture Associated Skin Damage (MASD) and other wounds, use of equipment and all aspects of managing incontinence. To be professionally and legally accountable for all aspects of your own work including management of patients in your care. To organise the job role effectively regarding clinical priorities and use of time. Act as a patient advocate through the application of critical and professional knowledge and skills. The post holder will require a high degree of communication skills in communicating highly sensitive information regarding individuals medical conditions. To ensure effective communication with the wider community and healthcare professionals, to persuade change, influence and improve practice. Empathy, reassurance and negotiating skills are required to encourage patient compliance to treatment/management strategies, where there may be significant barriers to understanding in some patient groups e.g. learning disabilities, mental health. Provide advice to palliative care services in wound and equipment management as appropriate. Promote better understanding on prevention and management of wounds, MASD and equipment provision to patients, their families and carers, and other agencies involved with the patient through education and support. Ensure personal clinical skills are regularly updated as required by Lead Clinician to a specialist level. Participate in research and clinical trials in accordance with Clinical Governance and Trust policy. Involvement of the service user, carers and other professional including clinical management plans where required. Person Specification Education / Qualifications BSc Nursing Tissue Viability Modules in the last 6 years Evidence of continuing professional development Willing to undertake further studies Teaching qualification Experience Level of experience of tissue viability within the present job Experience of working within a multi-disciplinary team Experience of participating to audit Development of clinical guidelines Knowledge & Skills Excellent verbal skills with awareness of the need for confidentiality at all times.
