Remuneration: $58.95 - $60.86 per hour + super + incentives Employment Type: Temporary Part Time until June 2026 Position Classification: Social Worker Level 3 Location: Taree Community Health Service Hours Per Week: 24 Requisition ID: REQ Applications Close: Sunday, 7th December 2025 Where You’ll Be Working Based in the Lower Mid North Coast Sector’s Transitional Aged Care Program (TACP), a dedicated service that helps older patients regain independence after leaving hospital. TACP brings together a team of experienced healthcare professionals to deliver tailored care that supports recovery and promotes wellbeing. About The Role Deliver high‑quality Social Work services, working alongside Allied Health and Nursing professionals. Key responsibilities include: Supporting patients and families through slow‑stream therapy, restoring function and boosting independence. Conducting home visits as part of a service operating daily from 8:00 am to 4:30 pm, 365 days a year. Receiving comprehensive onsite training, including CHIME training. Developing innovative strategies and solutions for complex patient needs. Participating in orientation, ongoing training, regular clinical supervision and peer support. About You We look for a dedicated, compassionate Social Worker committed to improving patient outcomes. You will bring: Self‑motivation, autonomy and strong time‑management skills. Genuine empathy, enthusiasm and flexibility. Excellent verbal and written communication skills. Commitment to safe, compassionate, person‑centred care. Requirements Bachelor’s degree in Social Work (or equivalent) ensuring eligibility for membership of the Australian Association of Social Workers. Unrestricted NSW Driver’s Licence and willingness to visit clients across the Lower Mid North Coast.
