Care Manager _Management

You are a member of the Village Leadership Team and you are a contemporary clinical leader. You are responsible for clinical care and clinical governance across the Village, working collaboratively to lead care teams in the delivery of our Sono Io model of person-centred care. Your clinical leadership and oversight of staff development achieves high-quality clinical care outcomes, including compliance with all relevant aged care legislation and Accreditation standards. YOUR OUTCOMES Person-Centred Care You model the Sono Io behaviours and lead care teams in the delivery of person-centred care. You achieve high-quality clinical care and clinical outcomes whilst ensuring that the resident is at the centre of all that we do. Systems & Processes You establish, maintain, and review clinical-specific strategies, policies, systems, and processes to support the village operations and all relevant aged care legislation and Accreditation Standards. People & Teams You lead and nurture a harmonious and cooperative team culture based on mutual respect and trust. YOUR ACTIVITIES Person-Centred Care You advocate for residents’ right to make decisions in all aspects of living. You connect with residents and families to form trusted relationships. You lead the clinical assessment, planning, implementation and evaluation of resident care; you oversee the development and monitoring of person-centred care plans. You notice and respond to any clinical issue that may affect the residents quality of life or ability to participate actively in what they enjoy. You provide expertise in specific clinical areas such as palliative care and acute care intervention. You participate in case conferences and care reviews, and you provide clinical leadership to the Multi Disciplinary Health Team. You liaise with medical practitioners and other external health care organisations to ensure effective care outcomes. Systems & Processes You manage clinical compliance and ensure all related accreditation outcomes can be demonstrated through documented evidence and work processes. You manage the resident admissions process and all operational activities relating to resident movements. You manage the continuous quality improvement program and clinical auditing processes. You oversee ANACC assessments to ensure maximum income is achieved; you contribute to the development and management of the clinical care budget. You support the effective utilisation of applicable information management systems. You ensure sufficient medical, pharmacy, and stock supplies. You follow established procedures for handling feedback or complaints from residents or families. Mandatory reporting applies to all allegations of elder abuse. You comply with all operational policies and procedures and applicable legislation. People & Teams You lead and nurture a harmonious and cooperative culture for the clinical care team based on mutual respect and trust. You manage and develop the performance of the clinical care team, and oversee the training and development of all staff. You recruit and select appropriate clinical and care staff; you oversee the staff rostering, including leave applications. You maintain and enhance your skills and knowledge through participation in education and professional development. You take care of your safety and the safety of others at all times.
