About the role The Senior Program Adviser supports on the delivery of the Victorian Community Health Seniors Program, which is funded under the Commonwealth's Strengthening Medicare – Supporting Older Australians (Victoria) initiative. The program is a pilot early intervention program targeted to Victorians aged 65 years and over to reduce frailty and decrease avoidable acute admissions with a focus on improving participants' functional capacity, enhancing wellbeing, and promoting independence. The role supports the design, development and implementation of the program, and its statewide delivery through community health services. The role will also provide sector and divisional direction of the program's day-to-day operations, whilst undertaking state-wide data reporting and performance monitoring to support continuous improvement of the model.
