Our modern social and affordablehousing features 84 one-bedroom villas for seniors over 55, over 45 for Aboriginal and Torres Strait Islander peoples, and 38 two-bedroom townhouses for single-parent families. Our team works together to provide secure, high-quality accommodation with integrated services that empower residents to live life their way. As a Tailored Support Coordinator , you will provide effective assessment and planning to coordinate and support tenants and household members access to a range of individualised services to improve their wellbeing and achieve their goals in life. The successful applicant will have: Certificate IV - Social Work or Community Services Experience in Social Housing, Community Services or related discipline Demonstrated ability to facilitate the provision of effective and individualised support to achieve positive outcomes for tenants and household members Experience working with high need, complex clients High level negotiation skills and ability to liaise with key stakeholders Competence in service assessment and documentation (including service planning) Demonstrate an understanding and appreciation of BaptistCare’s Christian identity and promote the organisation’s values through interactions at work Team player (cross functional team) Good working knowledge of Work Health and Safety practice Computer literate - intermediate to advanced MS Office and Outlook Good written and oral communication skills Tenacity, tolerance, empathy and patience with tenants and their needs Flexible and willing to work between locations as required Willing to take part in ongoing education A Valid NSW Driver’s License NSW Working with Children Check for paid employment purposes A current First Aid Certificate or willing to obtain If you're passionate about community support and want to be part of a team that values respect, inclusion, and empowerment, we’d love to hear from you About Us: We are proud to be part of the merged care organisation that will be known nationally as BaptistCare.
