Little bit about the role Based in St Kilda, Madden House is a 26-unit Community Housing residence supporting people who have experienced chronic homelessness or rough sleeping. In partnership with St Kilda Community Housing and Ngwala, VincentCare provides intensive, wrap‑around support to residents using a Housing First approach. The Case Manager will deliver responsive, person‑centred case management to help residents transition to and sustain long‑term housing. This includes addressing individual health and wellbeing needs, linking clients with appropriate services (mental health, AOD, disability, justice, family violence), and reducing the risk of returning to homelessness. The role involves planning, coordinating, and undertaking complex operational work with minimal supervision, while adapting support to meet fluctuating client needs. Key Selection Criteria Relevant tertiary qualification (or equivalent experience) in social work, community services, or related field; 1–2 years’ experience desirable. Valid Victorian Driver’s Licence. Understanding of professional client interaction, boundaries, and case management principles. Knowledge of issues impacting clients, including homelessness, financial hardship, family violence, mental health, substance use, and disabilities. Ability to provide holistic support, manage complex case plans, resources, and budgets. Commitment to social justice, human rights, cultural sensitivity, and trauma‑informed practice. Skilled in managing conflict and challenging behaviours. Strong organisational, time management, and communication skills, including report writing. Proficient in Microsoft Office and IT platforms; demonstrates initiative, sound judgment, and adaptability. Why work for us?
