Case Worker

Join the Luma team and play a key role as a Case Worker in our Active Recovery Team (ART). ART is a collaboration between North Metropolitan Health Services and Luma, providing coordinated support and treatment for individuals with severe and/or persistent mental health issues, who have multiple treatment and support needs. The focus for ART is to provide intensive psychosocial support to keep people well in the community. This is a part‑time role (0.6 FTE), fixed term for 6 months with the possibility of an extension. You will be working from our Wanneroo office, traveling to outreach appointments, networking events, and our other locations as required. Working with the support of the Team Leader and the wider team, you will be responsible for collaborating with North Metropolitan Community Mental Health Service to assist clients with emotional and practical support, case management, counselling, advocacy and referral pathways on an individual and group basis. Key Responsibilities Manage a client caseload, delivering case management, counselling, advocacy, and referral pathways for individuals accessing the service. Build a strong working alliance with clients from the outset of their involvement with ART. Conduct clinical assessments and develop tailored intervention plans based on individual client needs. Provide information, support, and referrals to help clients connect with the most appropriate services, within ART, across other Luma services, and externally when required. Promote hope for recovery, choice, and self‑determination, while equipping clients with skills and strategies to enhance their recovery experience. About you Tertiary qualification in Mental Health, AOD Counselling, Social Work, Psychology, or equivalent. Ability to deliver counselling interventions and advocacy for clients with mental health and/or AOD needs. Understanding of available services and ability to connect clients with appropriate supports. Strong written skills for case notes and reports; advanced verbal and non‑verbal skills for client engagement and case conferencing. Expertise in managing complex cases and trauma using sound clinical judgment and case management approaches. Why join the Luma team?
