POSITION DETAILS Title: Disability Support Worker Reports to: Client Services Coordinator Employment Conditions: Casual Location: 162 Grand Boulevard, Joondalup, WA Modern Award: Social, Community, Home Care and Disability Services Industrial (Casual) Performance Review: Annual PRIMARY PURPOSE As part of a team, carry out lawful duties, directions (as per individual duty roster), and group / program / individual activities aimed at supporting and assisting persons with a disability in their living and / or community environment. Staff are also to present a positive and professional image of the Association when interacting with clients, family members, advocates, service providers, funding bodies and the general community. JOB PURPOSE \To support people living in their own homes in their day-to-day living, ensuring that they have maximum opportunity for developing valued and valuing lifestyles. Support workers will be clear that their professional engagement with clients is concerned with generating quality personal relationships, which are supportive and life enhancing. They will provide opportunities for choice, self‑advocacy and participation in matters relating to the home as well as facilitating access to community services and facilities. KEY RESULT AREA / MAIN RESPONSIBILITIES Carry out designated duties/tasks as per the service duty roster, and other reasonable duties as instructed by the employer from time to time. Perform direct care duties for individual clients as specified in the Personal Care Profile, including but not limited to showering/bathing, dressing/grooming, toileting, preparing meals and assisting clients to eat their meals. Provide additional support such as gastrostomy feeding where appropriate training has been provided. Assist with clients’ physical and mobility requirements, including transferring, in accordance with training and workplace health and safety standards. Provide additional support such as personal administration, shopping, banking, paying bills, supervision of medication, washing and ironing of personal clothes and maintaining a clean living environment. Identify the need for, and support client to attend, medical/therapist appointments, and provide behavioural support. Transport clients as required. Facilitate community inclusion and participation, supporting clients to establish and maintain relationships and interests through access to community activities, venues and services. Carry out tasks and activities aimed at meeting client personal/living community needs, team objectives and program goals, consistent with CDSS’s model of practice. Promote positive routines and activities in a respectful, flexible manner that is meaningful to the individual. Encourage positive behaviour and manage challenging behaviour effectively. Promote empowerment and the right of choice within the community. Maintain appropriate client records and collaborate with clients, family members and stakeholders. Respond to crisis within specified guidelines and seek assistance if needed. Participate in developing and monitoring individual program plans and be accountable for their outcomes. Support and offer assistance to other support workers on the job training or orientation as requested. Maintain safe working conditions, report hazards and incidents promptly. Maintain current knowledge of organisational and departmental policies, procedures and guidelines. Embrace change and contribute to continuous improvement. QUALIFICATIONS & EXPERIENCE Sound experience, knowledge and/or training in disability or allied health/welfare/education relevant to the position. Current WA Driver’s Licence (essential). Current Senior First Aid Certificate (essential). Current Criminal History Screening or willingness to obtain (essential). Certificate in Disability Studies or related fields (essential). Compulsory third‑party insurance on private vehicle to be used for work purposes. General health & safety awareness. Minimum of one year’s driving experience and a full driver’s licence. SELECTING CRITERIA Demonstrated skills supporting and caring for adults with complex support needs, including challenging behaviours and medical supports. Ability to build a professional, supportive, encouraging and challenging relationship with clients. Experience working within a Lifestyle Planning process for persons with a disability. Ability to work autonomously and use initiative within CDSS policy and procedures. Demonstrated physical capacity to perform all duties. Excellent written, verbal and interpersonal communication skills, including conflict resolution, negotiation and problem solving. Willingness to work in diverse settings, including client homes and community settings. Knowledge and application of the principles and objectives of the Disability Services Act. Possession or willingness to obtain Senior First Aid Certificate and Manual Handling Certificate within 3 months probation. KEY BEHAVIOURAL INDICATORS Demonstrating leadership to achieve vision.
