If this sounds like you, we’d love to hear from you The Role The Wellbeing and Health In‑reach Nurse (WHIN) team is seeking experienced and motivated WHIN Coordinators (CNS2) to join our dynamic program. Based in Liverpool, this role provides the opportunity to work in a unique school‑based setting, delivering high‑quality, evidence‑informed, and culturally responsive care to vulnerable children, young people, and their families. You will work both independently and as part of a multidisciplinary team, fostering strong relationships across schools, health, and community sectors. This is a challenging and highly rewarding position that will further develop your clinical leadership, interagency collaboration, and community engagement skills. Key Responsibilities Lead and coordinate early intervention and referral pathways for school‑aged children and young people. Collaborate with school staff, health services, and community agencies to ensure accessible and responsive support for students and families. Build rapport and establish trust with children, young people and families to assess needs and support ongoing care. Promote holistic, integrated, and preventative care models. Actively contribute to strengthening links between Health and Education sectors. Current Authority to Practice as a Registered Nurse with Australian Health Practitioners Regulation Agency (AHPRA), with 3 years relevant Full Time equivalent post‑registration experience working with children and young people and approved postgraduate qualifications related to the specialty area of child and family health, or pediatrics, or adolescent / youth health nursing practice or in a field relevant to the specialty field, or such other qualification, or experience deemed appropriate by the organisation. Demonstrated experience working with children, young people and families, including marginalised and at‑risk populations with a proven ability to engage families in addressing health and mental health concerns using health promotion, prevention and early intervention approaches. Collaborative Practice, Leadership and Service Integration. Demonstrated ability to work effectively within interagency and multidisciplinary contexts, fostering collaboration, quality care and integrated service delivery. Proven ability to identify opportunities for enhancing care coordination and outcomes for children, young people and families. Applicants must possess an Australian C‑Class driver’s licence (including P1 or P2). Those who hold a C‑class licence (P1 or P2 included) from a state other than NSW must obtain a NSW C‑class (P1 or P2 included) licence within three months of appointment.
