About The Role The Change and Transformation Partner works within the Mission and Mission in Action of HammondCare and plays a key role in supporting the coordination, implementation and delivery of change and transformation initiatives. Key Focus Areas Process Evaluation & Improvement Change Management and leadership Education & Capability Building Stakeholder Engagement and Professional written communications to stakeholders Continuous Improvement & Insights Strategic Support & Partnership This is a permanent full‑time role based at St Leonards with hybrid opportunities. About You Commitment to engage and align with HammondCare’s Christian Mission, Motivation and Mission in Action. University level qualification in relevant health related discipline, business or leadership. Certification/Experience in Change Management Experience in aged care, home care, or community care, with a passion for client‑focused service delivery. Excellent communication and relationship‑building skills, with a commitment to advocacy and person‑centred care Knowledge of Aged Care Quality Standards and compliance requirements. Experience working collaboratively with healthcare professionals, carers, and external providers. Strong written and verbal communication, facilitation and stakeholder engagement skills Experienced in operating effectively in a multi‑site environment. A current driver’s licence and safe driving record. Willingness to travel between HammondCare’s sites and services. Excellent communication skills and ability to communicate with clients from diverse backgrounds Strong administrative and computer skills. Ability to work independently and as part of a team.
