About the Opportunity We have an exciting opportunity to make a difference to young people and their families and carers. Youth Health work with young people aged 12-24 who have multiple vulnerabilities and require support to access and navigate the health care system. Youth Health provides general, sexual and reproductive health services within a multidisciplinary team that includes psychology, OT, nursing, medical and Aboriginal health. We are a social work led service, and this role works alongside the Youth Health Senior Social Worker to provide clinical expertise and guidance to the team. You will work with key LHD and external partners to: Respond to young people with multiple vulnerabilities and/or chronic and complex healthcare needs as they access and navigate support services. Provide holistic psychosocial assessments and brief counselling to young people who require practical and emotional stabilisation to access health care and be partners in their own health care. Lead and support outreach activities to engage young people most at need. Actively build capacity in the community to cater for the unique psychosocial and health care needs of vulnerable young people and promote integration with other internal LHD and external services to ensure coordinated and integrated care pathways. About You This is an outstanding opportunity for someone who enjoys a diverse role. You have a bachelor’s or postgraduate degree in Social Work, along with eligibility for membership with the Australian Association of Social Workers (AASW), reflecting a comprehensive understanding of both generalist and specialist knowledge within the field. You will enjoy working within a multidisciplinary team, collaborating and partnering with key internal and external providers to ensure no wrong door entry for vulnerable young people into health care. You are an experienced clinician with expertise working with vulnerable populations. You will be comfortable providing clinical support to young people with significant complexity, and exercise independent judgment when managing cases. You will provide clinical supervision to level 1 and 2 social workers and clinical consultation to staff within the Priority Populations teams.
