As a Youth Care Coordinator, you will work collaboratively with a young person and their family/carers to identify their support needs, goals and develop collaborative care plans. The position involves supporting all young people who access headspace with a focus on supporting young Aboriginal and Torres Strait Islander people and fostering a culture of youth friendly, socially and culturally inclusive service experience. The Youth Care Coordinator Will Provide short term care coordination to young Aboriginal and Torres Strait Islander people and other young people and their families. Assess the mental health and psycho-social support needs of young people referred to headspace and coordinate transition to appropriate support and treatment streams within and external to headspace in accordance with the clinical need. Collaborate with local Aboriginal youth services to improve partnerships & access to support provided to Aboriginal and Torres Strait Islander young people and their families. This opportunity is available part‑time (22.5 hours per week) until 30 June 2026 with a possibility of extension. Hours and days are negotiable. Skills & Experience Identifies as an Aboriginal and/or Torres Strait Islander and is an active member within the local Aboriginal communities. Holds qualifications in a relevant field or working towards the same in youth work, community welfare, social work, psychology, peer work, occupational therapy, counselling or other related qualifications or demonstrated experience in a similar role. Can demonstrate an understanding of issues affecting the health and wellbeing needs of young people with mental health and/or substance use problems, as well as young Aboriginal people and their families. Has experience in delivering individual and group interventions.
