Northern Health are seeking applications from motivated Grade 3 Occupational Therapists to join our Occupational Therapy department – Full Time 1.0 EFT - Geriatric Evaluation and Management and Outpatient Renal (80 hours/ fortnight and monthly ADO) ongoing Northern Health is the major provider of acute, maternity, sub‑acute, mental health, specialist, community and home‑based services in Melbourne’s rapidly growing outer north. Services are provided through our five main campuses: Northern Hospital Epping, Broadmeadows Hospital, Bundoora Centre, Craigieburn Centre, and Kilmore District Hospital. The Occupational Therapy (OT) department at Northern Health prides itself on being inclusive, supportive and committed to evidence‑based practice. About the Role This role requires a highly proficient Occupational Therapist with experience in community and inpatient settings who is passionate for providing person‑centred care. You will work collaboratively with the multi‑disciplinary teams to facilitate timely assessment, interventions, and discharge plans that are dynamic and meet the needs and values of our diverse and aging population. This role is responsible for providing evidence‑based care that is in line with patient’s wishes, and supports access and flow throughout the health service. This role works with patients undergoing dialysis or living with Chronic Kidney Disease. You will be responsible for completing pre‑home dialysis assessments for both peritoneal dialysis and haemodialysis, as well as providing OT intervention to support patients to maintain their preferred dialysis modality. You will have active involvement in the day to day operations of both the Geriatric Evaluation and Management (GEM) at Kilmore and Renal Outpatient teams, and will work closely with the wider OT leadership team to drive service development and ensure a focus on innovation.
