What You’ll Do Supports families, and provides education and care for persons in their home or aged care homes Monitors and reports on the progress of clients and patient care plans Evaluates clients' needs and plans, and develops and implements educational, training and support programs Facilitates access to clinical services and health practitioners Interviews clients and assesses the nature and extent of their abilities Provides practical and emotional support to individuals Complies with relevant policies to optimise safety and wellbeing of clients What We’re Looking For Strong communication skills and a genuine interest in improving the lives of the elderly. Availability for regular shifts and a driver’s license with access to a vehicle. Must have a current First Aid Certificate, CPR Certificate, and relevant checks.
