You should be able to provide flexible, tailored support to meet the diverse needs of our clients. Responsibilities 1. Aged/Disabled Support Worker Provide individual support services and direct care. Assist with domestic activities and personal care, including personal hygiene and dressing. Assist with meal preparation. Support clients in community participation programmes. Assist with medication administration. Provide emotional support and companionship. Support individuals with complex behaviours of concern. Accurately document incident reports, progress notes, and shift handovers. Maintain records of client progress. Provide support to staff while performing shifts. 2. Personal Care Assistant Deliver comprehensive personal care services to clients in their own homes and community settings. Support clients with essential daily activities including personal hygiene, bathing, grooming, and assistance with dressing. Facilitate client mobility needs and provide communication support to enhance client interaction. Contribute to individualised care planning processes and implementation of care strategies. Assist with therapeutic interventions and specialised care approaches, particularly for clients with cognitive impairments and challenging behaviours. Monitor client health status and document significant changes in condition, escalating concerns appropriately. Facilitate basic rehabilitation activities and therapeutic exercises under professional guidance. Assist with administering prescribed medications and treatments according to established protocols. Maintain comprehensive records of care delivered and client responses to interventions. Ensure compliance with safety standards and quality care practices. Support clients with community participation and social engagement activities. Provide transportation assistance and accompany clients to appointments and community events. Adapt care delivery to the home environment while maintaining professional standards. 3. Nursing Support Worker Provide personal care under the direct supervision and guidance of registered nursing staff in clients' home environment and community settings. Assist patients with essential personal care activities including bathing, grooming, toileting, dressing, and maintaining personal hygiene standards. Support patients with mobility requirements, including transferring, positioning, and safe movement within care environments. Monitor and document patients' vital signs and other basic health observations as directed by nursing staff. Recognise and report changes in patients' physical and mental condition to nursing staff immediately. Participate in the planning and implementation of individualised patient care plans under nursing supervision. Provide communication support to patients and facilitate their interaction with healthcare team members and family. Assist registered nurses with clinical procedures and the administration of medications according to established protocols and guidelines. Support patients with rehabilitation exercises and therapeutic activities as prescribed by healthcare professionals. Ensure compliance with workplace health and safety standards, quality care practices, and organisational policies. Requirements Certificate III in Individual Support or related qualifications preferred. Related work experience preferred. Ability to work independently and effectively manage time. Strong verbal and written communication skills. Level II First Aid and CPR certification. Current WWCC/Blue Card (or willingness to obtain). Current Australian Driver's License preferred.
