You are the primary provider of personal care and support, and you strive for genuine connection with residents and their loved ones. YOUR ACTIVITIES Person-Centred Care You promote residents’ right to make decisions in all aspects of living. You connect with residents to form trusted relationships. You encourage family participation and positive engagement with the village community. You actively contribute to the assessment of resident care needs within scope of practice. You facilitate meaningful engagement for each resident; you provide personal care and support with daily activities in accordance with individualised resident care plans and any changing preferences or needs day to day. Care and support may include but not be limited to: lifestyle and social support, personal grooming and hygiene, dining support, medication assistance, simple wound dressing, routine health checks and observations, general domestic cooking and cleaning duties. You recognise changes in resident wellbeing and report to clinical support staff. You collaborate with the Care & Wellbeing Team and other internal and external support services to provide holistic and timely service delivery, with a focus on wellbeing. Systems & Processes You contribute to the development of flexible work schedules that respect the needs, choices, and preferences of residents. You complete relevant documentation and record keeping as per established guidelines. You participate in audits and continuous quality improvement processes. You adhere to manual handling procedures and safe use of all equipment; you report any safety issues or incidents. You follow established procedures for handling feedback or complaints from residents or families. Mandatory reporting applies to all allegations of elder abuse. You comply with all operational policies and procedures and applicable legislation.
