Case Worker - Permanency Support Program (Full Time - 12 Month Contract) Veritas House is a vibrant not-for-profit, community-based organisation with a specific focus on supporting vulnerable children, young people & their families. About the role As a Case Worker in our Permanency Support Program, you’ll play a vital role in the statutory out-of-home care (OOHC) system, working with children and young people who have experienced trauma, loss and instability. You’ll provide comprehensive case management to help them feel safe, valued and connected while working towards permanency and long-term stability. This is a challenging yet deeply rewarding role, where no two days are the same. You’ll collaborate with DCJ, carers, birth families, and specialist services to deliver the best-practice care planning principles that meets legal, emotional and developmental needs. You’ll also empower young people in Supported Independent Living to build the skills and confidence needed to transition successfully into adulthood. Specific responsibilities of this role include: Provide high-quality case management to children and young people in Foster Care and Supported Independent Living. Develop and implement case plans that promote safety, stability, and positive life outcomes. Support young people to build independent living skills, manage tenancies, and access education, training, and employment opportunities. Build strong, respectful relationships with children, young people, Foster Carers, and families. Collaborate with service providers and government agencies to ensure the best outcomes for each child or young person. Maintain accurate documentation and ensure compliance with Permanency Support Program and NSW Child Protection standards. Participate in the after-hours on-call roster and contribute to a positive, supportive team culture. Why work for us? So don't delay) Qualifications A relevant qualification in Community Services, Social Work, Youth Work, Case Management or a related field (minimum Diploma level). Experience in child protection, out-of-home care or youth services. Strong understanding of the Children and Young Persons (Care and Protection) Act 1998 and NSW Child Safe Standards. Excellent case planning, documentation and time management skills. The ability to engage effectively with children, young people, carers and families.
