Clinical Care Coordinator - Storm Village The Opportunity: Are you a dynamic and experienced Clinical Care Coordinator (CCC) looking to make a real impact? This is your chance to step into a pivotal leadership role at one of our vibrant Residential Aged Care Homes. Working alongside the Residential Care Manager, you'll help lead the clinical operations of the home, ensuring our residents receive exceptional care in line with the Aged Care Quality Standards. This is an ideal role for someone with strong change management skills and a genuine passion for uplifting people, culture, and care outcomes. Full-time, permanent role – may include occasional weekend work Based within our Residential Aged Care Home To be considered, you’ll need current AHPRA registration and proven clinical experience within the aged care sector. Access to Fitness Passport (over 300 gyms) Free access to our Employee Assistance Program 40% off school fees at Manning Valley Anglican College or any of our 4 Newcastle Anglican Schools Role & Responsibilities: Organising and facilitating case conferences to partner with consumers and their representatives in developing care plans; Be a clinical care leader and primary contact within the home; Provide expert case management alongside the Residential Care Manager; Monitor and improve clinical outcomes through audits and quality initiatives; Support policy development and implementation for resident care; Lead, coach, and inspire staff to deliver outstanding care. About you? Registered Nurse with current AHPRA registration; Proven clinical and leadership experience in residential aged care; Strong skills in assessments, care planning, and change management; Sound understanding of aged care funding models; Commitment to best practice and continuous improvement.
