Clinical Care Coordinator - Storm Village The Opportunity: Are you a dynamic and experienced Clinical Care Coordinator (CCC) looking to make a real impact? This is your chance to step into a pivotal leadership role at one of our vibrant Residential Aged Care Homes. Working alongside the Residential Care Manager, you'll help lead the clinical operations of the home, ensuring our residents receive exceptional care in line with the Aged Care Quality Standards. This is an ideal role for someone with strong change management skills and a genuine passion for uplifting people, culture, and care outcomes. Full-time, permanent role – may include occasional weekend work Based within our Residential Aged Care Home To be considered, you’ll need current AHPRA registration and proven clinical experience within the aged care sector. About you? Registered Nurse with current AHPRA registration; Proven clinical and leadership experience in residential aged care; Strong skills in assessments, care planning, and change management; Sound understanding of aged care funding models; Commitment to best practice and continuous improvement.
