As the Aboriginal Wellbeing Coordinator, you will be central to the delivery of coordinated, culturally safe, and patient-centred care that bridges hospital, virtual, and community-based services. This role supports the delivery of virtual care to enhance equity, access, patient flow, and service connection across the region. In this role, you will work Monday to Friday, 08:30am – 05:00pm. The successful applicant will support Aboriginal Health Practitioners and Health Workers in assessing and supporting Aboriginal people transitioning from acute care to community-based and out-of-hospital programs, work as part of a multidisciplinary team (MDT) to deliver coordinated care tailored to the needs of Aboriginal clients, facilitate timely and culturally safe access to health and social care services through expert care coordination and case management and promote equity by delivering virtual care solutions that improve service access and support continuity of care. You will be instrumental in reducing hospital length of stay, preventing unnecessary Emergency Department presentations, promoting safe, effective out-of-hospital care, strengthening continuity of care across health and social care systems, enhancing service integration to improve outcomes for vulnerable Aboriginal clients. This role forms a key part of the Western Virtual Connected Care in the Community – Planned Care for Better Health program. Your contribution will support the delivery of holistic, person-centred care that makes a meaningful difference to the health and wellbeing of Aboriginal people, families, and communities across Western NSW.
