An exciting career opportunity now exists for an experienced Registered Nurse Care Coordinator at our mixed-billing surgery located in Kurralta Park, South Australia. This is a part-time position at 0.6 FTE. As a Registered Nurse Care Coordinator, your primary role is to support our general practitioners and patients with chronic and complex health conditions. This includes conducting comprehensive health assessments, coordinating care, developing care plans, and assisting patients with navigating relevant health and community services. You will also be responsible for delivering high-quality, person-centred nursing care, including standard practice nursing duties such as immunisations, wound care and dressings, ear syringing, ECGs, assisting doctors with minor procedures, and other clinical tasks as required. The role requires a Bachelor of Nursing and current AHPRA registration. You will be a key member of our team, contributing to the efficiency of the Practice and supporting our doctors in delivering high-quality, individualised patient care. Skills and Experience: Current AHPRA registration GP Practice / Medical Centre experience is essential Knowledge of chronic disease management and health assessment is essential Capacity to multi task, prioritise and good time management Must be pro‑active with a can‑do positive attitude Professional presentation Strong verbal and written communication skills Current Working with Children Check Current First Aid and CPR Certificate Willingness to assist in Administrative tasks whenever needed Knowledge of cold chain management and the National Immunisation Program If you feel this is you and you meet the above criteria, we would love to hear from you and join our friendly team.
