About the Program Staying on Track is a Queensland Government initiative supporting young people transitioning from youth detention back into the community. Each young person receives up to 12 months of personalised support , including intensive mentoring, case management, and connection to education, training, employment, and cultural or community activities. The Youth Case Worker role can be based out of the organisation's Zillmere or Caboolture location. Key Responsibilities The Youth and Family Practitioner delivers holistic, trauma-informed casework and family support for justice-involved young people. This includes: Building relationships and co-developing Service Response Plans (SRPs). Providing intensive casework, advocacy, and wraparound support. Linking young people and families to education, employment, health, housing, and cultural supports. Supporting family-led decision-making and cultural connection. Participating in team meetings, multi-agency panels, and reflective supervision. This role involves community outreach, home visits, detention engagement, and occasional evening or weekend work to meet client needs. Role Requirements Availability to undertake on-call duties one weekend per month, with compensation via TOIL or an additional allowance. Experience in youth work, community services, casework, or related field. Knowledge of Aboriginal and Torres Strait Islander cultural frameworks and family systems. Skills in trauma-informed, culturally safe, and strengths-based practice. Ability to develop, implement, and review SRPs with young people and families. Strong communication, collaboration, time management, and record-keeping skills. Probity Requirements Minimum Certificate III in Community Services or related field Current QLD Driver Licence Possession of, or ability to obtain, a ‘Positive Notice’ Blue Card (Working with Children Check). First Aid and CPR certification, or willingness to obtain (the employer will cover this cost). Willingness to undergo a National Police and Criminal History Check (the employer will cover this cost).
