Employment Type: Full‑Time (Part‑Time also considered), Ongoing Salary: RPN Grade 4 + Superannuation + Salary Packaging About the role Western Health is seeking an experienced and motivated Team Leader to join our new and innovative Western Health Mental Health at Home (WHMH at Home) service – an exciting model of care delivering acute mental health treatment in the home environment. WHMH at Home is a 24/7 service providing the equivalent of ten acute mental health beds in the community under a Hospital in the Home model. As Team Leader, you will coordinate the day‑to‑day delivery of high‑quality, person‑centred care, providing clinical leadership, guidance and support to the multidisciplinary team while promoting safety, quality and recovery‑oriented practice. Key responsibilities Coordinate the daily operations of the WHMH at Home team, ensuring safe, high‑quality and consumer‑centred care. Provide clinical leadership, supervision and mentorship to nursing and multidisciplinary team members. Lead intake, allocation and review processes, ensuring effective triage and continuity of care. Support performance improvement, data review and audit activities aligned with Western Health’s Best Care framework. Foster a culture of psychological safety, inclusion, collaboration and continuous learning. Act as the senior clinician on shift and deputize for the Program Manager during periods of leave. About you You are a skilled and compassionate mental health clinician with strong leadership capability and a passion for recovery‑oriented, trauma‑informed care. You thrive in dynamic environments, value teamwork and are committed to delivering safe, person‑centred care in partnership with consumers, families and carers. Current registration as a Registered Nurse with AHPRA and postgraduate qualifications in Mental Health Nursing (or equivalent). Extensive clinical experience in acute and/or community mental health settings. Strong working knowledge of the Mental Health and Wellbeing Act 2022 and contemporary service frameworks. Proven ability to coordinate teams, manage workloads and make sound clinical decisions in complex situations. Excellent communication, problem‑solving and interpersonal skills, with the ability to inspire and support others. A commitment to continuous learning, reflective practice and evidence‑based care.
