Role Description This is a full-time on-site Case Manager role based in Wollongong, NSW. The Case Manager will be responsible for providing client-centered support services, conducting assessments, developing care plans, and coordinating resources to address the needs of clients. Duties include ongoing monitoring and review of client progress, collaborating with multidisciplinary teams, maintaining accurate documentation, and fostering strong relationships with individuals and families to achieve desired outcomes. The role will also involve navigating external support networks to ensure comprehensive care. Qualifications Strong assessment, case management, and care planning skills Knowledge of support services for vulnerable populations, including children, families, and people living with disabilities Proficiency in relationship-building, communication, and collaboration with community stakeholders Strong organizational and time-management abilities Understanding of compliance requirements and ability to maintain accurate client records Relevant qualifications in Social Work, Case Management, or a related field Prior experience in a case management or client support role is highly regarded A current driver’s license and willingness to travel within the service region #J-18808-Ljbffr
