Join us As a case manager, your contributions through effective case management will empower individuals receiving Commonwealth Home Support Program or Support at Home funding to maintain and support their independence, stay connected to their communities, and feel truly seen and valued. Responsibilities • Conduct comprehensive assessments and develop goal‑oriented care plans. • Coordinate with multidisciplinary teams and manage communications with consumers, families, staff, and stakeholders. • Engage and motivate participants to achieve personal goals while ensuring safe and compliant support delivery. • Use contemporary systems and technology to document and report case activity. Qualifications Certificate III Ageing, Certificate IV Nursing, Ageing/Aged Care or Disability (Registered or Enrolled Nurse highly desirable) Minimum 1 year experience in a similar role Sound knowledge and experience working with people who are ageing from an enablement, restorative and wellbeing perspective Understanding of multidisciplinary teamwork and ability to multi‑task Proven ability to conduct comprehensive assessments and develop goal‑orientated care plans Computer and technology skills to enable use of contemporary systems Skilled to engage and motivate consumers, families, staff and stakeholders About Us Formerly known as the Anglican Diocese of Newcastle, Newcastle Anglican is a faith‑based organisation serving the peoples of the Hunter and the Central Coast.
